Library Awards Nomination Form

Nominations for the 2009 Library Awards are now open! You can nominate library staff for the following Library Award categories:

  • Library Outstanding Contribution Award: recognises an outstanding and sustained contribution to a specific aspect of the library’s operations by a library staff member or team.
  • Library Service Award: recognises excellence in service by a library staff member or team.

Nominations can be made by University staff and students. Simply select a Library Award category from the drop-down list below and fill out the form.

Nominations close on the 1st of November, 2009.

All fields on the Library Awards Nomination Forms are Mandatory fields. Please ensure you have filled in each field before submitting your nomination.

The personal information requested assists with the integrity of the nomination process and will be kept in a secure database to be viewed by the Library Awards Administrator only.

Winners of the award will be chosen by the Library Management Group, and announced at the annual Library Awards function and in the Library News Service.
